Service Coordinators, or previously known as case managers, provide a variety of services to consumers with the aim of helping individuals achieve their specific wellness and recovery goals and objectives.
Services
The Service Coordinator can assist consumers in improving daily living skills, access to socially and supportive activities, medication adherence, provide short-term counseling, find appropriate housing, medical care or other treatment services in community.
Applying for aid from other agencies, such as Medi-Cal and SSI is also available. A Service Coordinator is offered to individuals who need this type of services.
Although consumers are not required to regularly meet with the Service Coordinator, this assignment assures that if a need for help comes up at any time, staff familiar with the individual's life will be able to assist them.
